October 1, 2007
Dear Patient:
I am pleased to announce my affiliation with the Harbor Medical
Group and Drs. Litoff and Carlson.
These experienced, dedicated and caring Board Certified internists
have been colleagues of mine for decades and I have worked with
them through the years. Our practice styles are similar.
It is with mixed feelings and a degree of sadness that I am announcing that as of December 1, 2007 I will no longer be seeing patients and will move into full time administrative and continuing teaching responsibilities. Drs. Carlson and Litoff will be taking over my practice of more than 34 years. I will greatly miss the privilege of taking care the many wonderful patients and friends I have made. I am extremely confident that your continuing care will be in the best of hands. Your medical records will remain in my new office and, of course, are available. Their schedules are such that they are able to accommodate urgent appointments. For those with mainly gastrointestinal issues they will be able to remain as your internist and we will be happy to refer you to one of my many gastroenterology colleagues for continuing sub-specialty care and endoscopic procedures. I have been unique in that I have given both sub-specialty and general internal medicine care
.
Dr. Carlson and Dr. Litoff are affiliated with Northwestern
University Medical School, where they remain active faculty.
They are also on the staff of Advocate
Illinois Masonic Medical Center. After I leave I will be available
to consult with them regarding your individual care. Under their
care you may retain your relationship with Northwestern specialists
and Northwestern Memorial Hospital if you desire. They are participants
in most insurance plans and HMO of Illinois.
I encourage you to make an appointment with me to conclude any medical
issues, complete yearly exams or discuss any other issues. You will
have an opportunity to be personally introduced to my colleagues
at that time. Harbor Medical Group is located in the same building
as my old office in suite 1710.
.
Yours truly,
Fred L. Fishman, M.D.
